A good rule also is the snakes girth at the widest point to be about the same as the girth of the widest part of the rat/mouse hips.
Has anyone ever had a mouse or rat get stuck in a ball python? I've heard so many variations of this rule from the size of its head to twice the size of its widest part. I'm just curious if anyone has actually had a problem with this.

Not sure, but wouldn't the snake just let it go if they can't fit it in their mouths after trying?
A BP is unlikely to kill anything it can't eat, or to eat anything it can't swallow. I was feeding WAY too small prey until I visited a breeder and watched them feed their snakes. I would have that snake on rats already.
