for underbelly heat, do you just put a UTH? Also the lid, do you put any holes or cut out an opening(add screen) to it.

For the belly heat, you can do a few things. You can either use some heat tape and use foil tape to tape it to the tub or you can buy a UTH. The most important part with either is that you MUST connect the heat source to a thermostat. This is very important because a lot of UTH and heat tape will get well over 100 degrees. The thermostat allows you to control the heat so the snake won't ever get burned.
For the lid and holes, you can also do a few things, you can cut a piece off the lid(not a big piece, maybe 1/3 of the lid) and secure a screen piece with some screws. That'll allow a lot of humidity to escape but not to much to were its bad for your snake. You can also drill holes all around the tub to allow the humidity to escape. Just make sure to test the humidity out a few days before putting the ball in. If the humidity is low, cover some holes up with clear tape. If you have too much humidity, drill more holes. To secure the lid on the tub so the snake won't escape, you can use bungee cords or binder clips.
You can get tubs at your local Target or Walmart. You can try office supply stores like Staples, Office Max or Office Depot. A lot of people on this site use the Iris brand tubs because they are really transparent. Most tubs that are sold do have lids but they aren't on the actual tub. They are on a self near the tub self, just look around when you go into the tub section of the store. If you haven't already bought your tub, i would recommend getting a 16 qt tub that has looking clips on the side. Hopes this helps.

Just thought I would add a tip for making holes in your tubs. I have found that using a drill Is messy, leaves rough edges, and sometimes cracks. So what I do actually is use an Ice Pick or old Phillips head screw driver and heat up the end in a flame (Stove, torch, ect.) and push it through the plastic gently. This works great for me, Makes nice clean holes with smooth edges and no plastic shavings to clean up after.

You could do this, or if you have an old soldering iron laying around, you can use that too My dad has like four of them and only uses one, so I grabbed his biggest one and used that on all my tubs.
