    Great news! Drove 45 minutes away to a more experienced reptile vet who managed to find what looked like it caused the problem. Turns out he had a ring of hair tightly wrapped around the base of his tongue that caused it to swell and not retract fully. The vet removed it, and he's now sticking out his tongue to sniff at things again and generally looking much happier and more relaxed.
