That can occur when the humidity in the enclosure is not high enough. What is the humidity at right now?

Those dents can also appear when there is a retained eye cap (ie: She didn't shed off her eye scales). When she last shed, did you check for both of her eye caps? If they were there, I would have to go with the humidity in the enclosure isn't just right.
