I'm not saying she's not underweight or malnourished. She could be both for all I know. It's hard to give a firm number of what a "normal" weight for a 4 year old bp would be. There are too many factors that come into play such as genetics and feeding habits, but could also be effected by illnesses like internal parasites.
Based on your description of the snake being weak and flat, it doesn't sound good. I would recommend starting off with smaller meals than you would normally feed. Start small and work up in prey size gradually as she puts on weight and regains strength and energy. I wouldn't put her on a 5 day schedule personally. Feed every 7 days, starting small and working up to appropriate sized prey and I'm sure you'll see the weight start to pack on.
