No it's not photoshopped in It's quite easy to achieve actually. The trick is to "make" your aperture opening, shapped like a star( or any shape infact).
To do this, you simply cover the front of your lens, I used black card board, and cut a hole at the middle of the cardboard, any shape you like
It's easier with lenses which have large aperture and large diameter, since you need the shape to be smaller than the lens' aperture, or else, the out of focus blur will take the shape of your lens' aperture which is round

After you have your "custom shaped" aperture, just find a background with a lot of highlight. I took the picture under a tree.
