I need to make some changes, I can't have this happening again, need to find the problem . Thinking of downsizing his enclosure as he never moves so think he a little scared in there , I'm not sure what sort of setup is best, housing , heat , substrate..... As must be something wrong , humidity or stress ?? Or do these things just happen for no apparent reason?

You could make your own tub, use paper towel as substrate, I made my own tub and constant kept good humidity and temps.
