Fill out application then send in with your test package
Check out what's available at

I filled out the application and sent it in with my test package taped to the outside of the test box. I then placed that package inside another larger package to be sent out.
It says to send package to this address, which is where I sent my package when I got certified just over a year ago. Once they received the package it took about a week for my rep to get the report. It was actually quite detailed with photos and experiments they performed to make sure the box would hold up.
Send your shipment to:
FedEx Packaging Services
2828 Business Park Drive
Bldg. H, Suite 500
Memphis, TN 38118
My advice is to send it exactly as you would with your live animal, minus the live animal. Include the delicup/bag, instructions for new owner, lable the contents of the package both common and scientific name, quantity and an emergency number for them to contact you in case of a problem. This will help you cover all the details. I would also place lots of tape on the outside of the box making sure it's very well secured.

Don't forget heat or cold packs as well.

Re: Getting fedex certified question
I wouldn't think you would need heat packs this time of year, and I never use cold packs. I did not include any heat or cold packs in my test package when I sent mine in, and it passed without them.

They will reimburse you for the shipping fee. I have heard that it is sometimes hard to get reimbursed, though. I just sent mine in ground, which only cost around six bucks, and didn't worry about getting reimbursed.

I meant heat packs if sending in test shipment for cold weather or ice packs for shipping in hot weather. I asked my red what to include and she said to package it exactly as you would minus the live reptile. So I did, I included a cold pack. Passed fine.
