I use the monthly Daytimer booklet I get w/ my daily daytimers every year.
I record Date fed, if ate and what, UR's and BM's, Sheds and how they went, Vet visits, any different or unusual changes in behavior or patterns and weights.
I usually take it to the vet with me when we have to go just in case.

Shoot I forgot to add on my comments about the Degei program that you can also print out cage cards with a picture on them too. Haven't tried this yet as I burnt out our printer awhile back and have yet to replace it.

I just bought one of those compostion books the kids use in school, and make a page for each month of the year, and then use each line for the day of the month, then write down what happened (if app.) on that day-who ate, what they ate, sheds, lock-ups,cage cleans, vet visits, egg lays, and any weights and lengths I happen to take. I have used this method for the past three years, and still haven't filled up the notebook. It is very useful to me, because when I want to do a comparison from year to year, or say, estimate how much weight someone gained, all I gotta do is flip back some pages..:)
