    Could be as simply as not spot cleaning - very easy to do; you just start spot cleaning :)

    Agreed. I'm pretty sure it was a result of overly wet substrate, though it seemed to show up pretty quick. I've been pretty good, though not perfect, about spot cleaning, but I have only had her since the beginning of December so her tank should still be fairly clean, even if I missed one or two. No shed box yet, but I will have to be careful when I add one for sheds.
