NEW POLL (hopefully) for all you smokers! (tabacco only)

edit: so i selected the number of options for the poll, and clicked "submit thread", and when its supposed to take me to "step 3" it was just a blank grey page

ive tried twice now to make a poll using this thread for reference:

and each time its supposed to take me to "step 3", it sends me to a grey blank page, is theres something im missing?:confuzd:

does anybody have any ideas?
