Fedex Test Package Question

I'm trying to get certified to ship snakes via Fedex and I'm shipping out the test package this week. I have 2 questions. First, is this test package application still valid http://www.fedex.com/us/services/pdf...pplication.pdf and should I tape it to the test package itself or to the "Over Box"

Yes the app is still valid and no just put the app in the over box with your test package
