Mostly bad husbandry from met moldy areas. Keep your cage nice and clean and your fine!

    Re: scale rot?

Too large of humidity (I'm having that issue right now. I have to melt new holes tomorrow because my poor ball's tub won't go below 80%).

Basically if your snake is on a wet substrate for a long period of time (including it's own urates/feces) it will receive scale rot.

Do your best to avoid it, as from what I hear, it's NOT easy or fun to treat.
