how do you modify links?

i see people on here that have a one word link, for example:
you can find the caresheet here, and the word "here" is the link to the caresheet page, instead of having the entire url typed out, can anyone explain to me how to do this please?

In the bar where you can "bold" and whatnot, there is a little picture of the Earth with a figure 8 symbol (next to the red X one). You copy the link you want to use, highlight the word you want to use, then click that button and paste the link
