I don't have any answers for your questions, but they all seem like issues the vet could advise on.

You don't mention any consideration of respiratory virus testing. Did the vet say anything about this?

The vet asked for an update in about a month when I've had a chance to see how he's doing post-antibiotics so I'll definitely bring all of that up with her. She didn't particularly touch on viruses and to my understanding the MiDOG test only covered bacterial and fungal infections, so testing for viruses is something I also want to ask about. To my understanding viruses can't be treated directly in the same way that bacterial infections can be, and he has been shown to improve with antibiotics so it would stand to reason that he does have a bacterial infection of some sort. That doesn't mean he couldn't also have a viral infection - perhaps the bacterial one could be keeping his immune system from being able to properly fight the viral one or vice versa? Certainly a possibility to consider!
